CLARIFICATION TEXT FOR SERVICE RECEIVER
PURSUANT TO
THE LAW ON THE PROTECTION OF PERSONAL DATA NO 6698 (LPPD)
This Clarification Text has been prepared for the purpose of fulfilling the clarification obligation within the scope of the law on the Protection of Personal Data No 6698 (“LPPD”) and other relevant legislation to be provided to the persons receiving services from the Data Controller Alindent Özel Ağız ve Diş Sağlığı Polikinliği at the address Muhsin Yazıcıoğlu Caddesi No: 8/1, Çukurambar/Çankaya/ANKARA Our Company’s Policies on the protection of personal data are available at the web address alindent.com.tr.
- PERSONAL DATA COLLECTION METHOD AND LEGAL REASON
In the capacity of data controller, it obtains personal data in writing, directly orally, through CCTV and medical devices from real persons receiving service. The personal data obtained is processed in line with the purposes specified in this clarification text and pursuant to the clause (c) of the second paragraph of Article 5 of the LPPD, “provided that it is directly related to the establishment or performance of an agreement, it is necessary to process the personal data of the parties of the agreement”, the clause (ç) “obligatory for the data controller to fulfil his legal obligations”, the clause (f) “data processing is mandatory for the legitimate interests of the data controller, provided that it does not harm the fundamental rights and freedoms of the data subject” and based on the express consent mentioned in the Article 6 of the LPPD.
- PURPOSES OF PERSONAL DATA PROCESSING
Processing of your personal data refers to all kinds of operations performed on the data such as obtaining, saving, storing, preserving, changing, reorganizing, disclosing, transferring, taking over, making available, classifying pursuant to the Article 3 of the LPPD. Your personal data processed by us, the purposes of processing this personal data and the legal grounds underlying the processing are detailed in the table.

- TO WHOM AND FOR WHAT PURPOSES PERSONAL DATA MAY BE TRANSFERRED
Our company pays strict attention to process your personal data in accordance with the “need to know” and “need to use” principles, by providing the necessary data minimization and taking the necessary technical and administrative security measures. Your personal data collected for this legal reason can be processed and transferred within the framework of the personal data processing conditions and purposes specified in the Articles 8 and 9 of the LPDD but may be transferred to the specified third parties in the cases specified below.

- RIGHTS OF THE RELEVANT PERSON AND METHOD OF EXERCISING THESE RIGHTS
Under the Article 11 of the Law on Protection of Personal Data, everyone has the right to apply to our Company, as Data Supervisor, regarding the following issues:
(1) Everyone has right to apply to the Data Supervisor and,
- a) To find out whether his/her personal data are processed,
- b) To request information regarding this if his/her personal data are processed,
- c) to learn the purpose of the data processing and if this data is used for intended purposes,
ç) To know about the third parties to whom personal data are transferred domestically or abroad,
- d) To request the rectification of the incomplete or inaccurate data, if any,
- e) To request his personal data to be erased or destroyed under the conditions stipulated in Article 7 of the Law No. 6698,
- f) To request notification of the operations conducted in compliance with subparagraphs (d) and (e) to third parties to whom his/her personal data has been transferred,
- g) To object to consequences to her/his detriment, arising from the analysis of the processed data exclusively via automatic systems,
ğ) To claim compensation in case of suffering loss due to illegal processing of the personal data
Pursuant to Article 4 of the Communiqué on the Procedures and Principles of Application to the Data Controller, titled “Right of Application”, natural persons whose personal data are processed have the right to apply to the data controller. Relevant persons may use this right provided that the make their applications in Turkish.
Applications regarding your rights listed above should be submitted to our company:
- By duly filling in the Application Form at Muhsin Yazıcıoğlu Caddesi No: 8/1, Çukurambar/Çankaya/ANKARA in the capacity of relevant person,
- Via alindent@outlook.com e-mail address, together with the documents identifying your identity, using your e-mail registered in our company’s systems, or
- By using other methods determined by the Board.
Depending on the nature of your request, your application will be concluded free of charge as soon as possible and within thirty days at the latest; however, if the transaction requires an additional cost, you may be asked to pay a fee according to the tariff to be determined by the Personal Data Protection Board.
